Quick answer: CalPERS’s fax for member forms is (800) 959-6545. It’s printed on forms such as the Member Change of Address (myCalPERS 2190), the Refund Direct Deposit Authorization (myCalPERS 1362) and the Active Employment Certification (myCalPERS 3003). Sign and date the form, put your CalPERS ID or Social Security number on it, and keep the fax confirmation page. For an address change, CalPERS recommends your myCalPERS account over the form, and if you’re still working, your employer updates your address with CalPERS instead.

The California Public Employees’ Retirement System prints the same fax number on its member forms, next to its postal address, P.O. Box 942715, Sacramento, CA 94229-2715. What matters is whether the form you’re holding can be faxed at all, and what CalPERS needs on it before it will act.

Before you fax a CalPERS form

CalPERS also warns that it may ask for extra validation before changing your details. That’s for the security of your account, so answer any follow-up quickly.

A new address can also mean a new health plan, because your CalPERS health plan has to cover your work or home ZIP code. Long-term care coverage is separate: update it with illumifin, which runs the CalPERS Long-Term Care Program, at (800) 982-1775.

When myCalPERS is the better route

For an address change, CalPERS recommends doing it in your myCalPERS account at my.calpers.ca.gov, because it’s processed faster than the paper form. Tax withholding changes can also be made there. Retirement applications can be completed in myCalPERS too, or on the hard-copy application from CalPERS’s Forms & Publications page.

If you’d rather talk to someone, CalPERS’s number is 888 CalPERS (888-225-7377), and its TTY line is (877) 249-7442.

Teaching in a California public school? Your pension is probably with CalSTRS, a separate system with its own fax numbers. See how to fax CalSTRS.

If you’re faxing the paper form

Municorn Fax sends the signed form to (800) 959-6545 and keeps the confirmation page with the date and time, so you can show when CalPERS received it. Add a front page with our cover sheet generator.

Frequently Asked Questions

Can I email CalPERS instead of faxing?

For the Active Employment Certification, yes: the form gives an email address for CalPERS’s refund unit, shown in the table above. The address change and direct deposit forms list fax and post only.

Where do I post a CalPERS form?

To CalPERS, P.O. Box 942715, Sacramento, CA 94229-2715, the address printed on the forms.

What’s the fastest way to change my address with CalPERS?

Your myCalPERS account. CalPERS recommends it over the paper form for faster processing.

Is CalPERS the same as CalSTRS?

No. CalPERS covers California public employees; CalSTRS covers California public school teachers. They have different fax numbers.
