Quick answer: Most MassHealth paperwork goes to the Health Insurance Processing Center at (857) 323-8300: new paper applications for help paying for coverage (the ACA-3), proof documents and renewal forms. Applications for seniors and people who need long-term care go to (617) 887-8799. Put MassHealth’s barcoded Health Coverage Mail/Fax Cover Sheet on top, and if a letter asked you for proof, fax to the number printed on that letter. Appeals are different: a Fair Hearing Request goes to the Board of Hearings at (617) 887-8797.
MassHealth has several fax numbers, and they map to who’s applying and what you’re sending rather than to which office you’ve dealt with. Massachusetts also shares its application with the Health Connector, so a few documents go to the Connector’s own fax. A letter’s own fax comes first: MassHealth tells you to mail or fax proof to the address or fax on the letter that asked for it.

The cover sheet
MassHealth’s Health Coverage Mail/Fax Cover Sheet has a barcode, and MassHealth asks you to use it whenever you mail or fax documents: the cover sheet plus the first page that carries the barcode, then your documents. Don’t reuse one two-page cover sheet for more than one household: each household needs its own.
If you’re returning an affidavit, such as the affidavit to verify zero income or Massachusetts residency, include a copy of the letter that asked for the proof. MassHealth calls that letter a Request for Information.
Appealing a MassHealth decision
Fill in the Fair Hearing Request form and send it with a copy of the notice you’re appealing. The Board of Hearings must receive your completed, signed request within 60 calendar days of the date you received the notice. Fax it to (617) 887-8797, email the form and notice to boh-fairhearingsrequest@mass.gov, or complete the request by phone through MassHealth Customer Service on (800) 841-2900.
Don’t phone the Board itself to file. Its numbers, (617) 847-1200 and (800) 655-0338, are for questions about a hearing. At least 10 days before the hearing, the Board sends you the date, time and place, and the hearing may be held by phone.
Old numbers on old paperwork
In 2019 MassHealth phased out the processing centre’s old fax, (617) 887-8777, and its old address, P.O. Box 1231, Taunton. Both still turn up on forms printed before then. The current fax is (857) 323-8300 and the current address is Health Insurance Processing Center, P.O. Box 4405, Taunton, MA 02780. An older edition of the ACA-3 is still accepted; just send it to the current number.
Appeals have the same problem. Some sites still list (617) 210-5820 for the Board of Hearings, a number from an old MassHealth provider directory. The current Fair Hearing Request form gives (617) 887-8797.
By post or in person
Post to Health Insurance Processing Center, P.O. Box 4405, Taunton, MA 02780, or hand documents in at a MassHealth Enrollment Center: Charlestown, Chelsea, Quincy, Springfield, Taunton, Tewksbury or Worcester. Not sure where something goes? MassHealth Customer Service is on (800) 841-2900 (TDD/TTY 711).
Municorn Fax scans the barcoded cover sheet and your documents in one go and sends them to (857) 323-8300 or the number on your letter, keeping a confirmation page as your record.
