Quick answer: Fax a request for hearing to the Oregon Workers’ Compensation Board (WCB) at 503-373-1600, and a request for Board review of a judge’s order to 503-373-1684. Include your name, address, employer, date of injury and claim number. Disputing a claim closure comes first, and goes to the Workers’ Compensation Division (WCD) at 503-947-7794 within 60 days. One document can’t be faxed at all: a Claim Disposition Agreement has to be filed by mail, through the WCB Portal or by hand.

Some sites list Oregon’s hearing fax numbers under the Workers’ Compensation Division (WCD). They belong to the Board. WCB holds hearings before an administrative law judge (ALJ), and the Board reviews those judges’ orders. If you’re challenging a denial or an order on reconsideration, the Board is who you’re writing to. A claim closure is different: it goes to WCD for reconsideration first, and only the order that comes back goes to the Board.

Disputing a claim closure: WCD first

If you disagree with your Notice of Closure, ask WCD to reconsider it using the Worker Request for Reconsideration (Form 440-2223a). Send it to the Appellate Review Unit within 60 days of the notice: by fax to 503-947-7794, by post, or by hand at WCD’s office in Salem. The fax limit is 20 pages, so split anything longer.

Treat it as your one chance. There’s only one reconsideration per claim closure, any issue you don’t raise in it can’t be raised in a later appeal, and it’s the last point at which new information can be added to the closure record, so send every document you want considered. If an attorney represents you, attach the signed retainer agreement. The Appellate Review Unit (503-947-7816) and the Ombuds Office for Oregon Workers (503-378-3351, or 800-927-1271 toll-free) can help with the form.

WCD then issues an Order on Reconsideration. If you disagree with that, the next step is a hearing request to the Board.

What a hearing request needs

Under Oregon law (ORS 656.283), a hearing request can be any writing, signed by you or for you, that gives your address and says you want a hearing. WCB’s form, the Request for Hearing and Specification of Issues (revised 10/2024), is still the easier way, because it lists the reasons you can tick. Either way, include:

The law requires a new hearing request to be scheduled within 90 days. A postponed hearing has to be rescheduled within 120.

Board review: copy everyone

A request for Board review only has to say that you want the judge’s order reviewed. Fax it to 503-373-1684, and send copies to every party in the case before the judge (ORS 656.295). Board review is normally scheduled within 90 days of the request, and the Board decides within 30 days after that. It can’t consider anything that wasn’t admitted as evidence at the hearing, so new medical reports won’t help at this stage unless the Board sends the case back to a judge.

On the last day, WCB also takes requests by hand at its staffed offices in Salem and Portland.

The settlement you can’t fax

A Claim Disposition Agreement settles your claim. The parties and their attorneys can pass copied, faxed or electronically signed versions between themselves, but filing the CDA with the Board has to be by mail, through the WCB Portal or by hand: WCB doesn’t accept a CDA by fax or email. Addendums are the exception, and those can go to 503-373-1684.

Without an attorney, you get a 30-day window to change your mind. The Board can’t approve your CDA before the 31st day after it receives it.

Email instead of fax

WCB takes hearing and Board review requests at request.wcb@wcb.oregon.gov. Attach the completed form and copy (“cc”) the other parties and their attorneys. A successful send gets an automated reply straight away; if none arrives, check the address and send it again. The WCB Portal is for registered attorneys only.

Before you hit send

When a deadline is close, a confirmation page for each fax matters: Municorn Fax sends the signed request to 503-373-1600 or 503-373-1684 from your phone and keeps one. The first single-page fax costs nothing.

Frequently Asked Questions

Is 503-373-1600 the Workers’ Compensation Division’s fax?

No. It’s the Workers’ Compensation Board’s fax for hearing requests. The Division’s fax for workers disputing a claim closure is 503-947-7794, for the Appellate Review Unit.

How soon will my hearing be scheduled?

The law requires a new request to be scheduled within 90 days, and a postponed hearing to be rescheduled within 120 days.

How do I postpone a hearing?

You need a good reason, such as having just hired an attorney. Ask the other party for their position first, then contact the Hearings Division in writing, by fax, by phone or online. Don’t assume the postponement is granted until the Hearings Division tells you.

Do I need a lawyer?

No. WCB points people to the Oregon State Bar Referral Service on 800-452-7636. Attorneys get a fee only if they win your case.
