Quick answer: Documents for a workers’ comp claim that L&I manages go to 360-902-4567, with your claim number on every page. On a self-insured claim, an address change goes to L&I’s Self-Insurance Program at 360-902-6900 and to your employer in writing. An Application for Claim Resolution Settlement goes to 360-902-5285. To start a new claim, use FileFast online or call 1-877-561-3453.
Who manages your claim decides the fax. If your employer is insured through the Washington State Department of Labor & Industries (L&I), L&I runs the claim and assigns you a claim manager. If your employer is self-insured, the employer or its third-party administrator (TPA) runs the claim, and L&I’s Self-Insurance Program oversees it.

Sending documents to an L&I claim
L&I asks for the claim number in the top right corner of every page of every document. It’s written for doctors’ offices; do the same on your own pages. Faxed pages go straight into the claim file, which is why L&I encourages faxing documents.
Bills are the exception. L&I doesn’t accept faxed bills and won’t process them, so post them to Department of Labor & Industries, PO Box 44269, Olympia, WA 98504-4269.
For questions rather than documents, your claim manager’s phone number is on every letter L&I sends you. The online Claim & Account Center lets you send your claim manager a secure message and read your claim file, and L&I’s automated claim line is 800-831-5227, in English or Spanish.
If your employer is self-insured
Your employer or its TPA handles the claim, so most paperwork goes to them. Address changes are the exception that L&I wants too: send written notice to your employer or TPA, and to L&I’s Self-Insurance Program by mail or by fax on 360-902-6900, with your L&I claim number.
Disagree with how the claim is being handled? Talk to your employer first. If that doesn’t settle it, call L&I on 360-902-6901, or the Office of the Ombuds for Injured Workers of Self-Insured Employers on 1-888-317-0493. Online claim files for self-insured claims carry less information than L&I’s own, so keep copies of everything you send.
Applying for a claim resolution settlement
A claim resolution settlement is open to injured workers aged 50 or over, once at least 180 days have passed since L&I received the claim and the order allowing it is final and binding. Fill in the Application for Claim Resolution Settlement (F240-002-000) and fax it to the Settlement Unit on 360-902-5285, or post it to PO Box 44251, Olympia, WA 98504-4251. The Settlement Unit’s phone number is 360-902-6101.
Our cover sheet generator adds a front page with your claim number, and Municorn Fax sends it with the signed page to 360-902-4567, 360-902-6900 or 360-902-5285. EveryFax covers faxing from a computer or email.

Frequently Asked Questions
How do I file a new L&I claim, and by when?
Online with FileFast, by phone on 1-877-561-3453 (1-877-561-FILE, 8 a.m. to 5 p.m., Monday to Friday), or on the Report of Accident at your doctor’s office. L&I must receive the report within 1 year of the injury, or within 2 years of the doctor’s diagnosis for an occupational disease. If your employer is self-insured, file the claim with your employer instead.
Can a closed claim be reopened?
Yes. L&I’s claims FAQ gives you up to seven years after the claim closes to apply to reopen it.
What does L&I need for an address change?
In writing by fax or mail, with your claim number and your signature, or online in the Claim & Account Center. On a self-insured claim, tell your employer or its administrator as well.
Where do I find my claim manager’s number?
It’s on every letter L&I sends about your claim. If you leave a message, your claim manager should call you back within 48 hours.
Who can I call about a self-insured claim?
L&I on 360-902-6901, or the Office of the Ombuds for Injured Workers of Self-Insured Employers on 1-888-317-0493.
