Quick answer: UCLA Health’s Release of Information fax is 310-983-1468. That’s the number for patients, and for third parties such as attorneys and insurers, sending a signed authorization. The STAT fax, 310-206-4831, is for physicians and hospitals only. Sign by hand: UCLA doesn’t accept electronic signatures, and unsigned or incomplete requests are sent back.

UCLA’s records page lists two fax numbers side by side. The STAT line serves doctors and hospitals that need records for urgent care. Everyone else, patients included, uses the Release of Information fax.

A handwritten signature, and every field

UCLA’s records page says electronic signatures are not accepted at this time, so a typed name or a signature drawn in a PDF app won’t do. Print the authorization, sign it in ink, then scan or photograph it to fax. The same page says unsigned or incomplete requests won’t be processed and go back to the requester.

Someone signing for the patient, such as a person named in an advance directive or a power of attorney, includes a copy of that legal document.

UCLA doesn’t fax records to a home

UCLA’s FAQ says it can’t fax records to home or business fax numbers. For your own copies, pick a delivery method other than fax.

Fees and timing

UCLA’s FAQ says requests are processed within 15 business days of receipt, and that there’s no fee for patients to receive their own records. The main records page also says UCLA will invoice you for producing records, with checks payable to UC Regents, but it doesn’t say which requesters that applies to. If you’re requesting for someone else, ask Release of Information on 310-825-6021 (weekdays, 8:00am to 4:30pm) before you send.

After you’ve signed the form in ink, Municorn Fax scans it with your phone’s camera and sends it to 310-983-1468. For other Los Angeles systems, see Cedars-Sinai and Kaiser Permanente, whose California regions take no faxes at all.
