Quick answer: Kaiser Permanente has no single records fax. Each region runs its own release-of-information office, and California has no fax at all: members there request records online at kp.org/requestrecords. Elsewhere it’s by region, for example Georgia (770) 220-3705, western Washington 877-848-6896 and Colorado 303-404-4750. The Mid-Atlantic region has a different fax for each medical centre. Find your region in the table below.

Kaiser is a set of regional health plans that share a name, and medical records are kept and released region by region. So faxing Georgia about a Colorado visit gets you nowhere. The region is wherever you were treated, which for most members is wherever their plan is.

Hawaii prints three numbers. The patient and third-party release form gives (866) 609-7402. The Hawaii records page lists (808) 432-5070 and (808) 432-4908, but only under pharmacy, insurance and government-programme records. For your own records or a release to someone else, we’d use the number on the form, and phone (808) 432-5092 if you’re unsure.

California: why there’s no fax

California’s authorisation form says outright: “Do not use for patient copies of or access to their medical records.” Patients go to kp.org/requestrecords instead, and in Southern California a detailed copy comes by secure email within five days. The form exists for the opposite case, letting a third party such as a lawyer or insurer request your records, and you give the signed form to them, not to Kaiser. It’s valid for six months.

Washington: rules worth knowing before you fax

The Washington authorisation is the most detailed of the regional forms. It limits each request to ten years of records. Teenagers sign for some of their own records: ages 14 to 17 for sexually transmitted infections including HIV, 13 to 17 for mental health and addiction services, and any age for reproductive care, so a parent’s signature alone won’t release those. Radiology images go to a separate Central Imaging Center fax, 855-524-2256. Records sent to a doctor or requested by a member are free, the region page says; other requesters may be charged under state rules.

How long it should take

Region pages quote anything from five business days (Mid-Atlantic) to 15 (Washington). The legal ceiling is the same everywhere: 30 days under HIPAA, with one 30-day extension if Kaiser tells you in writing why, according to HHS.

Sending it

  1. Find your region above, and download that region’s form from its records page. The forms differ between regions.
  2. Fill it in, sign and date it.
  3. Add a cover page with your name, date of birth, medical record number if you have it, and the page count. Our cover sheet generator makes one.
  4. Fax it to your region’s number, or your medical centre’s in the Mid-Atlantic, and keep the transmission report.

Without a fax machine, Municorn Fax sends to your region’s number, or your medical centre’s in the Mid-Atlantic, from your phone: scan the signed form with the camera and enter the number, and the app’s delivery report is yours to keep. There’s no charge for your first single-page test fax. EveryFax faxes straight from an email inbox and offers a two-week trial. Is there one Kaiser Permanente fax number for medical records?

No. Each region has its own, the Mid-Atlantic has one per medical centre, and the California regions have none. Use the table above.

I’m in California. How do I get my records?

Online at kp.org/requestrecords. In Northern California you can also post a written, signed request to Release of Medical Information, 25 N. Via Monte, Walnut Creek, CA 94598.

Does Kaiser charge for records?

It depends on the region. Washington says records requested by a member are free; the Oregon and southwest Washington page says electronic records are free and up to 25 printed pages are free. Georgia says some requests carry a fee.

Where do I send x-rays and scans?

Often somewhere else. Washington has a separate imaging fax (855-524-2256), and the Oregon and southwest Washington region asks you to request images from its imaging department on 503-571-8451.
