Quick answer: Fax an Aflac individual supplemental policy claim to 877.442.3522, with the claim form for your state and the supporting documents. New York policies use 877.844.0201 and group (certificate) claims 866.849.2970. An appeal of an individual claim decision goes to 888.659.1023, within 180 days.

Aflac’s supplemental policies pay benefits for events such as an accident, a hospital stay or a cancer diagnosis, and most claims come down to paperwork from a doctor or hospital. Where that paperwork goes depends on three things: whether the policy is yours or came through work, whether it’s a New York policy, and whether you’re filing a claim or disputing a decision.

Filing a claim: start with your state’s form

Aflac’s claim forms are state-specific, so pick the policyholder’s state of residence on its claims page before downloading one. Send the completed form with all the supporting documentation it asks for. Illinois life claim forms aren’t online; call 800.992.3522 and Aflac sends them.

Aflac’s HIPAA Claims Authorization to Obtain Information (form S-00216) lets Aflac request records from your doctors and hospitals. If your claim includes one, it goes to the same 877.442.3522 fax, so send the two together.

Disputing a claim decision

If you disagree with a decision on an individual supplemental claim, use Aflac’s Claim Appeal Form (HC0021). Say why you disagree, cite the policy provision that supports you if you can, and attach anything new from your provider: a hospital bill, office notes, an emergency room report. The deadline is 180 days from the claim decision date on your denial letter or Explanation of Benefits, and you can appeal up to three times per claim. Aflac aims to decide within 45 days once it has everything.

Keep appeals and new claims apart. The appeal form asks for one form per appeal, with no new claims attached.

Appeals on Aflac Life, Absence and Disability Solutions claims follow the instructions in your decision letter. Put your case number in every message, and in the subject line if you email myPLADSappeal@aflac.com.

With 180 days on the clock, the date the appeal arrived matters. Municorn Fax sends it to 888.659.1023 from your phone, with the camera scanning the form and the medical pages, and keeps a delivery report that shows the date. The app’s one free test fax covers a single page. Other insurers: Principal, The Standard and MetLife.
