Quick answer: Which fax you use for The Standard depends on the paperwork in front of you. Group claim packets print the fax at the top of each form: 800.378.6053 on the “Your Disability Benefit Claim” packet (SI 2047) and 971.321.8400 on the long-term disability packet (SI 3379). An individual disability claim goes to 971.321.5609, in New York too. Plans that came from Anthem Life send disability claims to 1-800-850-0017.

Group disability coverage through work comes with a claim packet from the employer, and The Standard’s contact block is printed at the top of each form. Both the phone and the fax in that block depend on the packet, so use the ones printed on yours. Forms The Standard mails you after you report an absence have their own return fax, in the table below.

The group packet: who sends what

In the SI 2047 packet, your employer fills in the Employer’s Statement first and mails or faxes it to The Standard before handing you the packet. You complete your own statement and sign the authorization. Your doctor completes the Attending Physician’s Statement and either gives it back to you to send with your forms, or mails or faxes it straight to The Standard. If more than one doctor treats the condition, each one completes a statement.

For absence and short-term disability claims, The Standard says a decision usually comes about a week after it has the completed application and all the documents it needs. If it can’t decide within a week, it tells you why. Benefits are paid weekly, after the waiting period in your group policy.

To send your part of a group claim to the fax on your packet, Municorn Fax scans each signed page with your phone’s camera and keeps the delivery report, which shows when The Standard received it. You can try the app first with one free single-page test.

Plans that came to The Standard from Anthem Life have their own claims team. Disability claims go to 1-800-850-0017 or AL-Claims@standard.com, or by mail to the Disability Claims Service Center, P.O. Box 2717, Portland, OR 97208-9830, and the claims phone is (844) 404-2111. Life claims on those plans go to 1-877-305-3901. If your plan documents name Anthem Life as the insurer, use these.

Individual disability: answer every question

The individual packet’s Insured’s Statement wants every question answered, with “NA” where one doesn’t apply, and a signature and date, or it’s returned. Add the psychotherapy notes authorization if you’ve seen a mental health provider, and copies of any Social Security, PERS or workers’ compensation decisions. Physicians are asked to mail their part of the Attending Physician’s Statement to The Standard directly.

New York policies come from The Standard Life Insurance Company of New York and have their own packet, but the fax is the same 971.321.5609. The phone numbers differ: 800.628.9797, or 800.378.6057 in New York.

HHS says disability insurance isn’t a health plan under HIPAA, so HIPAA’s privacy rules don’t cover your records once The Standard has them. The authorization you sign in the packet is what lets The Standard collect records from your doctors, so read that page before you sign it. Other insurers: Principal, Aflac and Lincoln Financial.
