Quick answer: MetLife has three claim faxes, and which one you need depends on where the policy came from. Group life (cover through an employer): 1-570-558-8645. Individual life (a policy bought directly): 1-908-655-9586, but only if the policy is under $100,000 or a certified death certificate is already on file. Disability claims: 1-800-230-9531.

If you’re not sure which kind of life policy it is, look at the paperwork. A group policy usually came through an employer or union and is in the employer’s benefits documents. An individual policy has its own policy number and statements in the insured person’s name. We checked every number on MetLife’s own pages on 15 October 2026.

Individual life: the $100,000 rule

MetLife’s claims page only offers fax, email and online for an individual policy when “the policy(ies) you’re claiming is (are) less than $100,000 OR the original/certified death certificate is already on file from another beneficiary”. Its claim form is blunter: over $100,000, “we require a certified death certificate”, which a fax can’t deliver. So for a larger policy, post the claim with a certified copy, to the address in your claim kit.

MetLife says two different things about the threshold. Its life claims FAQ says a plain copy of the death certificate is enough up to $250,000, while the claims page and claim form say $100,000. If the policy falls between the two, call 1-800-638-5000 before you fax, and ask which applies.

Each beneficiary files a separate claim form. Once MetLife has the claim, it says it reviews everything within five business days and comes back within ten business days if it needs more.

Group life: copies are fine, unless the death was abroad

For group life, MetLife’s claims page says you don’t need an original death certificate; a copy will do. The exception is a death outside the United States, where MetLife wants the original death certificate and documents showing the cause and manner of death, by post.

Group claims also involve the employer, and MetLife’s page says to contact the insured person’s employer for next steps. The group claims line is 1-800-638-6420, prompt 2.

Disability claims

MetLife’s forms library prints the disability fax, 1-800-230-9531, on each supporting form: the medical authorisation, the attending physician statement, the direct deposit form and the FMLA certification. Many employers file through MetLife’s MyBenefits portal instead; MetLife says online filing isn’t available to every group, and to call 888-608-6665 if you can’t use it.

Sending it

  1. Work out whether the policy is group or individual, and which fax applies.
  2. For individual life over $100,000, post it with a certified death certificate instead.
  3. Put a cover page on top with the insured person’s name, the policy or claim number, your name and phone number, and the page count. Our cover sheet generator makes one.
  4. Fax the claim form and documents together, and keep the transmission report.

For a claim that can go by fax, Municorn Fax sends it from your phone: scan the claim form and documents with the camera, enter the fax for your policy type, and keep the delivery report the app produces. There’s one free single-page test fax to try first. EveryFax does it from your email inbox and has a two-week trial. Other insurers: Unum and Lincoln Financial.
