Quick answer: Fax the completed, signed Medical Records Release Form to Henry Ford Health on (313) 916-3917, or email it to HFHSMedicalRecords@HFHS.org. Henry Ford says processing can take up to 30 business days. Treated at a former Ascension hospital or office? Newer visits are in MyChart, and older ones still need the form or the online request.

Henry Ford Health is folding former Ascension hospitals and care sites into its own system, so where your records live depends on the date of your visit as much as on the place. The release form and the fax are the same for every Henry Ford hospital except the behavioral health hospital in West Bloomfield, which runs its own records office.

Former Ascension locations: MyChart or the form

For records from former Ascension locations, Henry Ford gives two cut-off dates. Physician office visits from 8 November 2025 onward, and hospital stays from 6 June 2026 onward, are in MyChart. Anything earlier needs the paper release form, faxed or emailed, or the online Henry Ford Medical Record Request, which asks for an email address, a cell phone and a photo ID. The records line for former Ascension hospitals is (800) 367-1500; for Henry Ford’s other hospitals and care sites it’s (313) 916-4540.

MyChart questions go to (313) 876-7951.

What the release form asks for

A signed authorization is valid for a year unless you write in another expiry date. If you’re asking to inspect records in person rather than get copies, the form must reach Henry Ford within 60 days of the date you signed it.

X-ray films and imaging CDs

The medical records department can’t provide radiology films or CDs. Ask the radiology department or the facility that did the scan instead.

Before faxing (313) 916-3917, check that the form names the location, provider and dates of service. Municorn Fax sends the signed pages from your phone and keeps a delivery report as proof of the date you sent them. For other Midwest systems, see Cleveland Clinic and Mayo Clinic.

Frequently Asked Questions

What does Henry Ford charge for copies?

Fees may apply, its page says. The form waives the fee when records go straight to a treating physician or healthcare facility.

Is emailing the form safe?

Henry Ford accepts the signed form by email at HFHSMedicalRecords@HFHS.org, but its form warns that email sent over the internet may not be secure. Fax or mail avoids that: Henry Ford Health Information Department, 1414 E Maple Road, Troy, MI 48083.

Who can see my Henry Ford records besides me?

Anyone you authorize on the release form. A representative who signs for you must include a copy of the guardianship papers or power of attorney.
