Quick answer: Mayo Clinic Release of Information takes faxes on 507-284-0161. Use it for a records request you’ve put in writing, or for the signed form that sends records to a third party (MC0072-01). Phone 507-284-4594 if you’re unsure. For your own records, the Patient Portal is usually quicker, and Mayo says it doesn’t charge you for your own copies.

Mayo runs one Release of Information team for its records, and its medical records page lists four ways to reach it: phone, fax, email and post. We checked the numbers on that page on 15 October 2026.

Your own records, or someone else’s?

This decides which paperwork you fax. Mayo’s form, the Authorization to Release Protected Health Information to a Third Party (MC0072-01), is for sending records to someone other than you: its own instructions name an insurer, an employer or a legal purpose, and say it isn’t for a family member or friend. For your own copy, use the portal, or contact the team with a written, signed request, by fax if you like.

Either way, the request needs to be in writing and signed. Federal guidance from the Department of Health and Human Services confirms a provider may accept “a faxed or mailed copy of a signed request” when you direct your records to someone else.

What the third-party form asks for

A patient aged 18 or over signs for themselves unless they’re incapacitated or have died. The form expires a year after you sign unless you write a different date. It also comes in Arabic, Somali, Hmong and Spanish versions.

Don’t use this number to send records to Mayo. If you’re sending another hospital’s records ahead of a Mayo appointment, Mayo asks you not to send them to Release of Information. Send them to your care team through a Patient Portal message instead.

How long it takes, and what it costs

Mayo says requests are “typically processed within two weeks of receipt”. It also points out the federal limit: under HIPAA, a provider has up to 30 days, and the HHS guidance allows one further 30 days if it tells you in writing why, within the first 30. On cost, Mayo’s page says there’s no charge for you to receive your own records, though it may charge “reasonable, cost-based fees” for copies where the law allows. The third-party form says copies may be charged under state law.

Imaging is handled a little differently. You can ask for images through the portal or in writing to the same team. They come as a cloud link by email, on an encrypted CD by post at no charge, or by FedEx for a $45 flat fee and delivery within two business days.

Sending the fax

  1. Fill in the request or MC0072-01, then sign and date it.
  2. Add a cover page with your name, date of birth, Mayo Clinic number if you know it, and the page count. Our cover sheet generator makes one.
  3. Fax it to 507-284-0161 and keep the transmission report.
  4. If nothing has happened after two weeks, phone 507-284-4594 with the date you sent it.

Mayo’s 2019 edition of the form lists separate Release of Information fax numbers for Arizona, Florida and the Mayo Clinic Health System in Minnesota and Wisconsin. The current web page lists only the Rochester number above, so use that one.

You don’t need a fax machine to reach 507-284-0161: Municorn Fax sends the request from your phone once you’ve scanned the signed pages with the camera, and it gives you a delivery report to keep. Sending your first single-page test fax costs nothing. EveryFax, the email-to-fax option, sends from your inbox and has a two-week trial. For how faxing medical records works in general, see how to fax medical records, and for other health systems, Cleveland Clinic and Kaiser Permanente.

Can I fax Mayo Clinic to get a copy of my own records?

Yes, with a written, signed request to Release of Information on 507-284-0161. The MC0072-01 form is for third parties, so you don’t need it for your own copy. The Patient Portal is the other route, and often the quicker one.

Does Mayo charge for medical records?

Its page says there’s no charge for you to receive your own records, but that it may charge reasonable, cost-based fees for copies where the law allows. Copies released to a third party may be charged under state law.

How do I get records for my child?

Mayo’s page says you can request a proxy access account to see the electronic records of your child, or of someone you’re the legal representative for.

Can I email instead of faxing?

Yes. Mayo lists MAYOHIMSROI@mayo.edu for Release of Information alongside the fax and phone numbers.
