Quick answer: For Cleveland Clinic’s Ohio facilities, fax a signed authorisation form to Health Information Management on 216-587-8043 (weekdays; after 5pm and at weekends, 216-445-6782). Akron General, Union Hospital and the three Florida hospitals have their own numbers, listed below; send it to the wrong one and it waits in the wrong queue. If you have a MyChart account, requesting through it is free.

Cleveland Clinic is really several records offices under one name. The main Ohio team covers most of the system, but some hospitals it acquired still run their own release-of-information desks and their own forms. So start with where you were treated.

Where to fax, by hospital

The main form also names Ashtabula County Medical Center as one that handles its own requests. We didn’t find its fax number on a Cleveland Clinic page, so call that hospital directly.

The form, and what makes one get sent back

For Ohio, the form is the Authorization to Disclose Health Information, revised 11 September 2026, also in Spanish. Older copies are still floating around on the site (a heart centre version is dated 2015), so download it fresh. It asks for your name, medical record number if you know it, address, phone, email, date of birth and the last four digits of your Social Security number.

Three things on it get forms returned:

Psychotherapy notes need a separate authorisation. The form expires a year after you sign it.

Florida asks for ID. All three Florida hospitals want every field completed and a copy of your signed photo ID with the form. Union Hospital asks for photo ID too. Put it behind the form in the same fax.

Fees and timing

Cleveland Clinic publishes its fees. Records through MyChart are free, and so are records sent directly to your healthcare provider. By email it’s a flat $6.50 plus tax. Paper copies for patients, parents and guardians are 36 cents a page for the first 50, 26 cents up to 200, and 12 cents after that. A third party pays more: an $18.91 search fee, then $1.24 a page for the first ten.

The two Cleveland Clinic pages disagree slightly on time: “7-10 business days” on one, “up to 10 days” on the other. Either is inside the federal limit, which is 30 days with one 30-day extension if the hospital tells you in writing why, per HHS. Radiology images use a different request form and take about 48 hours.

Sending it

  1. Check where you were treated, and use that hospital’s fax from the table.
  2. Complete every field on the right form. Sign and date it.
  3. Add a copy of your ID if it’s Florida or Union Hospital, and any representative paperwork.
  4. Put a cover page on top with your name, date of birth and the page count. Our cover sheet generator has a box for it.
  5. Keep the transmission report. If you’ve heard nothing in ten business days, call the number in the same row.

Once you know which hospital’s number to use, Municorn Fax can send the form from your phone, with your ID copy if you need one: scan them with the camera, type in the number and hold on to the delivery report. One single-page test fax is free to start. From an email inbox, EveryFax does the same, with a two-week trial. Also see how to fax medical records, and the Mayo Clinic and Kaiser Permanente guides.

What is Cleveland Clinic’s medical records fax number?

216-587-8043 for the Ohio facilities, on weekdays. After 5pm and at weekends it’s 216-445-6782. Akron General, Union Hospital and the Florida hospitals have their own numbers, listed above.

Can I request records through MyChart instead?

Yes, and it’s free. Cleveland Clinic says records requested through MyChart can take up to three business days to release, and that this isn’t available in the MyChart mobile app.

Can my child’s records go to my MyChart?

Cleveland Clinic says a child’s records can only be sent to an adult’s MyChart, and asks parents to contact the right records team for help with minors.

Do I need the hospital’s own form?

For the Ohio facilities, use the Authorization to Disclose Health Information form revised in September 2026. Akron General has its own form, and the main form says some hospitals handle their own requests directly.
