Quick answer: It depends on the state you were treated in. Intermountain Health’s Utah and Idaho records team takes faxes on (385) 215-7047; the team for Colorado, Montana, Wyoming and Nevada uses (303) 467-8966. Both fill requests within 30 days. Use the patient request form for your own copy and the authorization form to send records to someone else.
Intermountain releases records through two regional offices rather than one. Picking the right one means going by the state of the hospital or clinic, not where you live now. Someone who moved from Denver to Salt Lake City and wants their Colorado records still faxes the Broomfield team.

Two forms: yours, or someone else’s
The Patient Request for Health Information form is for getting your own records. It asks you to fill out every section to avoid delays: your name now and at the time of treatment, date of birth, phone and address; the hospital, clinic or doctor; the dates of service and types of record; how you want them delivered; and your signature with the date and time. It comes in Spanish too.
To have records sent to another person or organization, such as a new doctor or an attorney, use the Authorization for Disclosure of Protected Health Information instead. Legal, insurance and law enforcement requesters can skip the fax entirely and use Intermountain’s third-party request portal.
MyChart and walk-in requests
If you have a MyChart account, go to Menu, then Sharing, then Request Records; Share My Record sends records to someone else. Parents and legal guardians of children aged 12 and under ask for access through Request Access to a Child’s Record in the same menu. You can also hand the form in at a medical records department in person.
Charges may apply for copies and for radiology images on CD or DVD, and records sent to someone else can carry extra fees where state law allows.
Once you know which office has your records, Municorn Fax sends the form to (385) 215-7047 or (303) 467-8966 from your phone, with no fax machine needed. EveryFax sends the same form from your email. For other systems in the West, see Kaiser Permanente, which has a Colorado region of its own, and Baylor Scott & White.
