Quick answer: Mass General Brigham handles records requests for most of its hospitals in one place: the Release of Information Unit in Somerville, fax 617-726-3661. Corrections to your record are a different job with a different fax, 857-282-8110. Don’t email a records request; Mass General Brigham asks you not to, and there’s no in-person pickup.

Mass General Brigham is a group of hospitals, and each one has its own version of the release form, but the signed forms all go to the same unit. A handful of affiliated providers keep their own records desks, so check the list below before you send anything.

Hospitals the Somerville unit covers

The central unit takes requests for Brigham and Women’s Hospital, Brigham and Women’s Faulkner Hospital, Massachusetts General Hospital, Mass Eye and Ear, McLean Hospital, Newton-Wellesley Hospital, Salem Hospital, North Shore Medical Center’s Union Hospital, Cooley Dickinson Hospital, Nantucket Cottage Hospital, Wentworth-Douglass Hospital and the Spaulding locations. If your records are spread across several of them, use the system-wide form rather than one hospital’s.

These providers handle their own requests, and Mass General Brigham links to their websites instead of giving a fax:

Fields the unit needs filled in

Mass General Brigham lists these as required: the patient’s full name, including any maiden name; address and phone number; email address; date of birth; medical record number if you have it; the date of service; and the provider, facility or clinic the records come from. Add where the records should go (mail, fax, email or Patient Gateway), then sign and date the form. Specially protected or privileged information may need a further authorization.

For a minor, or an adult who can’t consent, a parent, guardian or other legal representative signs, with proof of their authority if it’s needed. For a patient who has died, the executor or administrator of the estate authorizes the request and sends proof of that role. Unless you write another date, the authorization expires six months after you sign it.

Timing, fees and scans

Mass General Brigham says certain information can take up to 30 days to process, and that fees may apply to some requests under state and federal rules. The form flags copying fees for insurance and legal purposes in particular.

X-rays and other images come from a separate Image Service Center, and the web page and the form give different contact details for it. Call Release of Information Customer Service on 617-726-2361 to confirm where to send an imaging request.

All the hospital versions of the form go to 617-726-3661, so Municorn Fax only needs that one number: scan the signed form and send it. EveryFax does the same from an email inbox. See also Duke Health, Cleveland Clinic and how to fax medical records.
