Quick answer: Patients fax a signed request to Tampa General Hospital on (813) 844-1135, its dedicated patient fax line. Third parties use a different form and a different fax, (813) 844-5891. Neither number is on Tampa General’s medical records page: each is printed on its own form. TGH Brooksville, Spring Hill and Crystal River have records faxes of their own, listed below.
Tampa General’s records page explains how to request copies, but it leaves the fax numbers to the forms, and the two forms send you to different lines. Which one you need turns on who is asking and, for the three hospitals north of Tampa, where you were treated. A patient, or the parent of a minor, uses the patient form. Anyone else requesting with the patient’s permission uses the third-party authorization.

The patient form
Form A503PT asks for your name, date of birth, the last four digits of your Social Security number, a phone number and the dates of treatment, then your signature and the date. Tampa General’s page says every request must be in writing.
Others who can request a patient’s records: the parent of a child under 18 who isn’t emancipated, a healthcare surrogate or someone holding power of attorney, with ID. For a patient who has died, records go to the administrator of the estate.
Records come back by mail or MyChart
Tampa General only faxes medical records to other medical facilities, for immediate patient care. Your own copies come another way, and there are no walk-up requests or in-person pickups. In MyChart, open the Sharing Hub and submit the Request formal copy of health record form; that’s for the patient or a proxy with an active account.
Cost and timing
Copies usually cost something, Tampa General says, with two exceptions: records released straight to your MyChart account, and records faxed or mailed to a verified physician’s office. Requests are processed within 30 days, and the form warns that this can be extended by another 30.
Using the patient form? Its line, (813) 844-1135, takes the signed pages straight from Municorn Fax on your phone, and the app’s delivery report shows when your request arrived. Florida patients of Cleveland Clinic will find its Weston, Martin and Indian River numbers in our guide, and Duke Health covers North Carolina.
