Quick answer: Lincoln Financial uses different faxes for different claims. Group life through an employer: (800) 462-4660. Group disability: (877) 843-3950. Annuity death claims: 260-455-9419. That last one is not a life insurance fax. For individual life policies, Lincoln publishes no general fax; it’s post or phone.

Lincoln’s claims go to different places: workplace claims to Omaha, individual life to Greensboro, annuities to Fort Wayne. Each has its own forms, and most have their own fax. Workplace forms live on lincoln4benefits.com, which is where lincolnfinancial.com/claimforms sends you. We checked each number on Lincoln’s own forms on 15 October 2026.

Group life: the certified death certificate

Lincoln’s group life claim form (GLC-01253) is unusually firm: “All Life Claims must be accompanied by a Certified Death Certificate.” A fax only ever delivers a copy. So the fax number is useful for the claim form and the employer’s section, but the certified certificate itself has to reach Lincoln on paper. Phone (800) 423-2765, ask whether they’ll open the claim from a faxed form while the certificate follows by post, and note who you spoke to. The form also lists LifeClaims@lfg.com for submitting claims.

The employer has a part to play: Lincoln asks it to complete its section and include the enrolment form and any beneficiary changes or assignments.

Group disability

Lincoln’s one-call claim flyer has you start a short-term disability claim by phone, on 888-480-8710, with forms faxed to (877) 843-3950. The long-term disability claim form (GLC-01252) uses the same fax and has three sections: the employer’s, yours and your doctor’s. Your doctor is asked to attach office notes from the last two years, which is usually the bulkiest part of the fax.

Annuity death claims

For an annuity or annuity-based IRA, the claimant’s statement (CL06541, revised 11/24) goes to 260-455-9419 or DeathClaimForms@lfg.com. A copy of the death certificate is enough if the contract or death benefit is under $1,000,000; over that, Lincoln wants the original, by post to Fort Wayne.

Individual life policies have no published fax. Lincoln’s individual claim forms give a postal address in Greensboro and the claims phone line, 800-487-1485 (option 4). Call first; if they give you a fax number on the phone, write down the name of the person who gave it. A fax number on a specialist form (the foreign death questionnaire) isn’t one we’d use for an ordinary claim.

Sending a Lincoln claim by fax

  1. Check which claim type you have, and use that row’s fax.
  2. Put a cover page on top with the insured person’s name, the policy or group number, your name and phone number, and the page count. Our cover sheet generator makes one.
  3. Send everything for one claim in one fax, and keep the transmission report.
  4. For group life, arrange for the certified death certificate to go by post.

Municorn Fax can send a Lincoln claim from your phone instead of a fax machine: scan the pages with the camera, enter that claim type’s fax number and keep the app’s delivery report. Your first fax can be a free single-page test. EveryFax covers sending from an email inbox, with a two-week trial. Other insurers: Unum and MetLife.
