Quick answer: Unum’s claims fax is 800-447-2498 for group claims through an employer: life and AD&D, short-term and long-term disability. Individual disability policies use a different number, 1-866-562-4794. Unum asks you to allow 14 days for faxed claims to be processed and says filing online is fastest if your employer’s plan allows it.

Most Unum claims come from cover you have through work, and those all share one fax line. It’s printed on Unum’s contact page under “Can’t file online?” and at the top of each group claim form. We checked the current forms, all revised in 2025 or 2026, on 15 October 2026.

Which Unum fax, for which claim

From Unum’s current claim forms.

Claim Form Fax Who fills it in
Group life and accidental death (AD&D) CL-1091 (05/26) 800-447-2498 The beneficiary and the employer
Group long-term disability CL-1019 (05/26) 800-447-2498 You, your employer and your doctor, each faxing their own section
Group short-term disability CL-1104 (07/25) 800-447-2498 You, your employer and your doctor
Individual disability (a policy you hold yourself) CL-1020 (05/26) 1-866-562-4794 See the form; it asks for your name and date of birth on every page

Life claims: a faxed death certificate is fine

Unum’s group life claim form asks for “a copy of the certified death certificate, if available (a photocopy or fax is acceptable)”; MetLife and Lincoln want certified copies in some cases. With it, send the beneficiary’s statement; the employer sends its own statement, the enrolment records and the latest beneficiary designation. For an accidental death claim, add the police report and, if there was one, the autopsy and toxicology reports.

You don’t have to wait until you have everything. The form says a claim can be started with the employer’s statement alone, and the beneficiary can send the rest separately. We’d put the insured person’s name and date of birth on every page, and the claim number once you have one, so separate faxes end up on the same file.

Disability claims: three people, one fax number

A group disability form is split into an employee section, an employer section and an attending physician’s section, and each person can fax their own part straight to Unum. That’s practical when your doctor’s office would rather fax than hand you clinical notes. For long-term disability, Unum also wants a job description, three months of payroll records, and the doctor’s office notes and records.

Every page of Unum’s disability forms has a space for your name and date of birth at the top. Fill it in on every page, including your doctor’s. Loose pages without it are hard to match.

Individual policies are different. If you bought the disability policy yourself rather than through work, use the individual claim form (CL-1020) and fax 1-866-562-4794, not the group number. If your short-term disability is individual only, Unum doesn’t need an employer statement.

Online first, if you can

Unum’s claims page calls its web portal and app “the fastest way to file a claim or leave”, and you can photograph documents and upload them. Online filing depends on your employer’s plan, so check with HR. Fax is the fallback Unum offers alongside post (P.O. Box 100158, Columbia, SC 29202), and it’s the route your doctor will probably use anyway.

After you fax

Allow 14 days, which is the figure Unum puts next to the fax number. Keep your transmission report. Unum’s claim lines are 866-779-1054 for leave and disability and 800-445-0402 for term life and AD&D.

Municorn Fax sends your part of the claim from your phone: scan the pages with the camera, enter 800-447-2498, or 1-866-562-4794 for an individual disability policy, and keep the delivery report the app returns. The app lets you send one single-page test fax free. EveryFax is the email-inbox route, and it has a two-week trial. Other insurers: MetLife and Lincoln Financial.
